Print, complete and mail your completed Order Form and payment to: Ms. Miller Hats, 6825 Theall Lane, Suite B, Houston, Tx 77066. Include your telephone number on the Order Form in case we need to contact you concerning your order, and include an email address (optional) if possible. Money orders must be in US currency and made payable to our affiliate: Miller Hats.
Personal Checks must include a printed name and street address, (not a PO Box and no temporary checks), and made payable to our affiliate: Miller Hats. To ensure immediate processing, include your Drivers License number on your check. Checks received without necessary information can delay processing of your order for as long as 14 business days.
